If you do not see your insurance plan listed below, call us at (808) 622-4942 and we can verify our participation
with your plan. If you are unsure of your insurance coverage or plan, please call your insurance company to confirm. If you do not have insurance coverage or choose not to use your insurance, we may be able to work out a reasonable payment. We strive to verify your insurance coverage and inform you of your copay in a timely manner. Copayments are expected to be collected at the time of service once the cost is established. We accept cash, check, and all major credit cards for copayments.
